Process:	Complete nomination forms Return completed forms to Ministry of Youth Development and Sports or Youth and Sports officers in the communities
Requirements:	Nominees must be between the ages of 10 – 35 years Both individuals and groups can be nominated Nominations must come from National Youth Organizations, Schools, District Youth and Sports Councils and Youth and Sports Officers A passport size photograph of the person being nominated must accompany all forms along with a testimonial from the organization or individuals doing the nominating
Notes:	Successful applicants must make themselves available for preparation meetings and to participate in the awardees program
